We focus on removing environmental barriers that impact mobility, daily functioning, and overall quality of life.
Our team, along with your PCP, coordinates individualized assessments and recommended modifications based on each member’s medical needs, functional limitations, and living situation. By working closely with healthcare providers and support systems, we ensure that each modification is appropriate, effective, and aligned with the member’s care plan.
These improvements help reduce the risk of falls and injury, support greater independence, and promote long-term housing stability by enabling individuals to safely remain in their homes.
