Blissful Horizon Foundation's Enhanced Care Management (ECM) program provides personalized support for individuals with complex health and social needs. Our multidisciplinary team of nurses, Community Health Workers (CHWs), social workers, and case managers works together to help members navigate the healthcare system, coordinate services, and access the resources needed to improve their overall well-being.
Using a whole-person approach, we connect members to medical care, behavioral health services, housing supports, community resources, and other essential programs. Whether managing chronic conditions, transitioning from a hospital stay, or overcoming barriers to care, our team provides ongoing guidance and advocacy every step of the way.
