Beyond the basics, our team provided her with personalized case management, healthcare coordination, and transportation support to medical appointments.

The case management team helped her navigate the complex process of obtaining vital documents—identification, medical records, and benefit eligibility paperwork—that had been lost or scattered during her time on the street. They connected her with housing resources, helped her understand her options, and advocated on her behalf with housing agencies and landlord partnerships.

Over several months of intensive support, Maria successfully transitioned into permanent supportive housing. The team didn't disappear after placement; instead, they maintained contact to ensure she had everything needed to thrive in her new home.
