Blissful Horizon Foundation's Recuperative Care program provides a safe, supportive environment for individuals experiencing homelessness who need a place to recover after a hospital stay, illness, injury, or surgery. Our program bridges the gap between hospitalization and long-term stability, allowing participants to heal in a comfortable setting while receiving personalized support.
What sets our program apart is our integrated approach to recovery. Participants benefit from clinical oversight provided by our in-house mobile physician team, ensuring medical needs are monitored and coordinated throughout their stay. In addition to medical support, we provide case management, care coordination, transportation assistance, benefits navigation, and connections to community resources.
Recovery is only the beginning. From admission, our team develops a comprehensive exit plan focused on each participant's long-term goals. Through housing navigation, resource linkage, and ongoing support, we help individuals transition from recuperative care into stable housing and a healthier future.
